I certify that I am an officer or an authorized representative of the above named organization and that I have authority to reserve the Computer Learning Center . I hereby certify that the statements made above are true to the best of my knowledge.

• I agree that I will be responsible for any damage to the library’s premises, furniture or equipment as a result of the organization’s use of the Computer Learning Center and I agree to pay for such damages assessed by the Library Board of Trustees
• I have read and agree to abide by and uphold the Library’s Computer Learning Center Use Policy. Furthermore, I certify that the organization’s use of the Computer Learning Center is in compliance with such policy. I understand that failure to follow the Computer Learning Center Use Policy may result in loss of Computer Learning Center privileges.
• I agree to indemnify and hold harmless the Kenton County Public Library, the Board of Trustees, the Library Director, their agents and employees from and against any and all loss, cost, expense, liability or damages caused by the organization, its officers and agents as a result of its use of the Computer Learning Center.
• I understand that the Computer Learning Center is not available to me until this application has been approved by the Library.
